In the spotlight // Amherstburg Family Health Team (AFHT)

Optimizing Healthcare for the Residents of Amherstburg.

The Amherstburg Family Health Team (AFHT) moved into their new building at 721 Front Rd. S. in November 2013 having operated out of the Seasons Retirement Home location since 2007. Our new 6800 sq. foot facility has seen a steady increase in patient registration. The AFHT recently welcomed Dr. Heba Youssef to the team and is looking forward to welcoming Dr. Jennifer Bondy (with local ties) at the end of October.

The five physicians work closely with a team of medical professionals to collaborate and provide patients with the optimal health care they deserve. In addition to the five physicians, patients will benefit from the expertise of a nurse practitioner, two registered practical nurses, one COPD/asthma educator, one nutritionist, one social worker and one health/diabetes educator.

The AFHT offers a number of programs in a group setting. Current and upcoming programs are:

  • The 3 C’s of Healthy Relationships: Communication, Compromise & Conflict Resolution
  • Grief Support Counselling
  • Chronic Pain Management Support

A family health team (FHT) is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for their patient. To enhance patient services we run a blood draw clinic on Tuesday and Thursday from 8:30 am to 12 pm and have a pharmacy on site (First Choice Pharmacy).

The AFHT is currently accepting new patients. AFHT patients also benefit from patient-only after-hours clinics on Monday  and Wednesday evenings and Saturday mornings. Visit www.afht.ca for more information.
